This form must be completed for any student who wishes to participate in an 850 activity. This one form covers all 850 events and trips, unless an additional waiver is required by a specific venue, organization, or partnering ministry.

This application is for students who are interested in taking a next step in leadership with 850. Our Student Leadership Team is designed for students who want to grow in their faith, serve others, help lead by example, and be part of what God is doing through this ministry. Completing this form helps us get to know each student and prayerfully determine if this is the right next step for them.
